Pledge Forms

Three different forms are available, each with a specific target pledge audience
in mind.  

Sponsor Ad Form - Designed for a single contributor to get their information on
the bench.  This form would be used for businesses, alumni, and anyone else
that contributes to your Bonkers® Fundraiser.

Pride Statement Form - Designed to be used by your members to get their
names on the bench with their accomplishments.  The form allows members to
collect multiple pledges making it easier for them to reach their fund raising goal.

Combined Spaces Form - Designed to allow combining of sponsor spaces for
multiple people or groups of people.  Examples include full team rosters, a list of
club members of a particular year or event, a list of State qualifiers of a particular
year, relay teams and their accomplishments.  Any combination of sponsor
spaces can be done.  i.e. 2 spaces wide by 3 high gives six spaces for that
group.  That group should raise $600.00 or more for those spaces as to maintain
the $100 per sponsor space minimum.
Helpful information:

1) It is recommended that
each Sponsor Space be
required to raise a
minimum of $100.00.

2) The cost to have each
Sponsor Space engraved
is $25.00.
